Home Health RN Job Details

• Weekly Estimated Pay: $2,120–$2,230
• Location: Denver metro area, Colorado
• Healthcare Setting: Home health
• Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m., with a rotating on-call schedule
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• Contract Duration: 13-week contract

• Additional Information: Mileage between patient visits is reimbursed

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license
• Associate degree in nursing or higher
• At least 1 year of home health nursing experience
• Experience completing OASIS assessments and start-of-care documentation
• Strong clinical judgment, organization, communication, and patient education skills
• Ability to travel throughout the Denver metro area and participate in a rotating on-call schedule

Home Health RN Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Denver metro area
• Complete start-of-care visits and OASIS assessments accurately and thoroughly
• Deliver wound care and monitor patient progress
• Manage medications, laboratory needs, and catheter care
• Develop, implement, and update individualized patient care plans
• Educate patients and caregivers on disease management, medications, treatments, and safety
• Coordinate care with providers, caregivers, and the broader interdisciplinary healthcare team
• Meet productivity expectations of approximately 30 points per week
• Respond to rotating on-call needs and support continuity of care
