Travel Hospice RN Job Details

• Location: Clarksville, TN and surrounding service areas
• Weekly Estimated Pay: $2,120–$2,230
• Assignment Type: 13-week travel nursing assignment
• Start Date: November 9, 2026
• End Date: February 8, 2027
• Schedule: 8:00 a.m. to 4:30 p.m., with on-call requirements
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week
• Patient Volume: Approximately 7–8 patients per day, with volume subject to census and daily needs
• Coverage Area: Multiple counties across the service region; assignments may vary based on patient census and operational needs
• Travel Support: Mileage reimbursement is available for eligible travel between patient visits

Hospice RN Job Requirements

• Current, active, and unencumbered Tennessee RN license or compact RN license
• At least 1 year of hospice or home health nursing experience
• Associate degree in Nursing required
• Experience with HCHB charting or a comparable homecare documentation system
• Ability to work independently in patients’ homes and collaborate effectively with physicians and interdisciplinary care team members
• Strong organization, prioritization, communication, and time-management skills
• Willingness to participate in shared on-call coverage

Travel Hospice Registered Nurse Responsibilities

• Assess patients and develop, coordinate, and manage individualized hospice care plans
• Monitor changes in patient condition and communicate clinical needs promptly to physicians and the interdisciplinary team
• Perform medication reconciliation, coordinate medication refills, and reinforce medication-related education
• Complete accurate and timely clinical documentation in the HCHB charting system
• Prepare weekly interdisciplinary group notes and participate in weekly interdisciplinary group meetings
• Provide compassionate end-of-life care and support to patients and their families in the home setting
• Manage a daily schedule across assigned service areas while adapting to changing census and patient needs
• Participate in on-call coverage alongside other members of the hospice care team
