Job Requirements

• Current Registered Nurse (RN) license
• Associate degree required
• Hospice nursing experience required
• Point-of-care experience required
• Professional certification required
• Strong communication skills and a compassionate, team-oriented approach
• Ability to support rural patients and families with extra care and attention

Responsibilities

• Provide high-quality nursing care in a hospice and homecare setting
• Manage patient cases and coordinate admissions
• Deliver compassionate support to patients and families throughout the care journey
• Complete point-of-care clinical tasks and documentation accurately
• Support coverage needs as needed for PTO or sick calls
• Work collaboratively with the interdisciplinary care team to ensure smooth, patient-centered service
• Travel within the assigned coverage area to meet patient care needs
