- Travel Radius: Up to 75 miles from the assigned care center
- Patient Load: Approximately 5-8 patients per day
- Productivity Expectation: 30 points per week
- Call: Potential rotating on-call coverage
- Schedule Notes: No guaranteed hours; overtime must be pre-approved
- Charting System: HomeCare HomeBase (HCHB) and PointCare; OASIS proficiency required
- Other Details: Local candidates accepted; mileage reimbursement available
Job Requirements
- Current Registered Nurse (RN) license in hand
- Minimum 1 year of adult home health experience or 1 year of hospice experience, depending on specialty
- Recent experience with HomeCare HomeBase required; OASIS proficiency required for RNs
- Current CPR/BLS certification
- Reliable transportation, valid driver’s license, and auto insurance
- Comfortable traveling daily to patient homes and attending periodic office meetings
- Willingness to cover a broad territory and manage a dynamic home care schedule
- Ability to complete documentation within 24 hours of each visit
Responsibilities
- Provide skilled nursing care in patients’ homes according to physician orders and agency policies
- Use the nursing process to assess, plan, implement, and evaluate patient care
- Coordinate and direct care delivery to support safe, high-quality outcomes
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as needed
- Complete Start of Care, wound care, follow-up visits, and other home health services
- Travel between patient visits while maintaining timely charting and visit documentation
- Work collaboratively with the care team to support patient needs and continuity of care
- Maintain professionalism, flexibility, and strong communication throughout the assignment
