- Travel: Must be willing to travel up to 75 miles from the assigned care center
- Charting: HomeCare HomeBase experience required; OASIS proficiency required for RN candidates
- Dress Code: Scrubs
Job Requirements
- Active Registered Nurse (RN) license in hand
- Minimum 1 year of adult home health experience or 1 year of hospice experience
- Recent specialty experience required
- Current CPR/BLS certification
- Proficiency with HomeCare HomeBase EMR within the last 6 months
- OASIS proficiency required for RN candidates
- Reliable transportation, valid driver’s license, and active auto insurance
- Comfortable driving to multiple patient homes daily and attending weekly office meetings
- Willingness to complete documentation within 24 hours of each visit
- Open to on-call rotations and variable weekend coverage as needed
Responsibilities
- Provide skilled nursing care to patients in their homes based on physician orders and agency guidelines
- Coordinate, direct, and document home health services using the nursing process
- Develop and follow individualized care plans to support patient recovery, safety, and continuity of care
- Complete visits, travel, and documentation efficiently while maintaining high-quality patient care
- Assess patient needs and respond appropriately to changes in condition
- Supervise and evaluate home health aides and LPN/LVNs as assigned
- Manage a varied caseload that may include start-of-care, wound care, and follow-up visits
- Maintain punctual, accurate charting and strong communication with the interdisciplinary care team
- Travel throughout the assigned territory and remain flexible to patient scheduling needs
