Job Requirements
- Current RN license in the state of Montana
- 1+ year of recent acute care, home care, or hospice nursing experience
- BSN preferred
- Valid driver's license
- Appropriate auto insurance
- Reliable transportation
- Experience with venipuncture, catheterization, wound care, and/or parenteral therapy preferred
- Strong communication, documentation, and care coordination skills
Responsibilities
- Provide skilled nursing care and case management for patients in their residence or inpatient hospice setting
- Admit patients to services and coordinate care plans with the interdisciplinary healthcare team
- Assess, observe, and evaluate patient physical and psychosocial status, including changes in condition
- Plan and implement nursing interventions based on physician-authorized care needs
- Oversee care delivered by Home Health and Hospice Aides and CNAs
- Promote continuity of care through effective communication with other disciplines and community resources
- Provide teaching, counseling, symptom management, and appropriate referrals to support quality of life
- Maintain accurate and timely patient records in accordance with agency standards and OASIS requirements
- Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
