- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Weekly Estimated Pay: $1,830-$1,930
- Productivity Expectation: 30 units per week
- Travel Requirement: Up to 75 miles from the assigned care center
- Weekly Meetings: Periodic office meetings required
- Guaranteed Hours: Not guaranteed
- Dress Code: Scrubs
Job Requirements
- Active Registered Nurse license in hand
- Minimum 1 year of Adult Home Health experience or 1 year of Hospice experience, depending on specialty
- Recent experience in the specialty required; first-time travelers are not accepted
- Current CPR/BLS certification
- Proficiency in HomeCare HomeBase (HCHB) within the last 6 months, or willingness to complete retraining if needed
- For RNs, proficiency in OASIS within the last 6 months, or willingness to complete retraining if needed
- Reliable transportation, valid driver’s license, and auto insurance
- Comfortable driving to patient homes daily and traveling between visits
- Ability to work rotating weekend coverage and on-call as assigned
- Strong time management, organization, and clinical documentation skills
Responsibilities
- Provide skilled nursing care to patients in their homes based on physician orders and agency policies
- Coordinate and direct home health services in collaboration with the interdisciplinary clinical team
- Complete assessments, start-of-care visits, wound care, follow-up visits, and other home health services as needed
- Supervise and evaluate home health aides and LPN/LVNs when applicable
- Maintain accurate and timely OASIS and clinical documentation within 24 hours of each patient visit
- Manage a daily caseload of approximately 5-8 patients and meet weekly productivity expectations
- Travel throughout the assigned territory and remain flexible to accept scheduled patient visits
- Participate in periodic office meetings and required rotations
- Deliver compassionate, patient-centered care while supporting safe outcomes in the home setting
