Key Responsibilities- Manage and perform the full admissions process, including documentation and coordination of new resident intake
- Facilitate transfers and discharges in accordance with facility protocols
- Complete fall reporting and follow appropriate incident response procedures
- Administer medication pass at the start of the night shift
- Oversee and coordinate care delivery in collaboration with CNA staff (up to 2 CNAs on nights)
- Responsible for all resident treatments during the shift
Required Qualifications- Active Registered Nurse (RN) license in good standing
- Experience with admissions, transfers, discharges, and fall reporting in a long-term care or skilled nursing setting
- Ability to manage a resident caseload of up to 35 residents per shift
- Must live at least 80 miles from the facility to be eligible for standard contract rates
Preferred Experience / Skills- Prior experience in a long-term care or skilled nursing facility (LTC/SNF) environment
- Familiarity with med aide coordination and bath aide scheduling workflows
- Strong organizational skills with the ability to oversee multiple CNAs and ancillary staff
