Registered Nurse – LTC / Health Home Management (Assessment RN)

Schedule: 5x8 Days (8:00 AM–4:00 PM / 9:00 AM–5:00 PM, flexibility required)
Guaranteed: 40 Hours/Week

We are seeking an RN with Home Health, LTC, or similar experience for a Health Home Management role focused on patient assessments. This is a non-bedside position with no hands-on patient care.

Requirements:

• Active Massachusetts RN License
• BLS Certification
• Minimum 1 year of recent RN experience
• Experience with MDS or OASIS assessments (required)
• Reliable transportation
• Willing to float to all CHL locations (up to 38-mile radius)

Responsibilities:

• Complete patient assessments in the home and/or office
• Develop and update care plans
• Medication reconciliation
• Work alongside a care partner who knows the patient
• Documentation for insurance/program requirements

Notes:

• Home Health, LTC, or similar assessment experience accepted
• Psych experience is a plus, not required
