Job Title: Registered Nurse Profession: RN Specialty: Transitional Care Duration: 13 weeks Shift: 12H Days Hours per Shift: 12 hours Experience: Minimum 2 years License: Required Certifications: BLS Certification Must-Have: No false nails Description: The facility provides the right place for many older patients to receive supervised restorative care after hospitalization or an acute illness. It offers the right care at the right time when the patient has begun recovery but is not yet well enough to be home. The skilled nursing facility bridges the gap between high-tech hospitalization and family home care. The focus is on geriatric nursing. The goal is to improve the patient's health skills and ability to function within the community. Each patient receives optimal care from a full range of health care disciplines. Disciplines include medicine, nursing, and respiratory therapy. These contribute to the patient's progressive restoration to the highest achievable level of functioning. The unit has a total of 32 beds. The staff-to-patient ratio ranges from 1:9 to 1:12. A charge nurse is present who handles all admissions and physician orders. Nurse aides are available with a ratio of 1:12. The unit orientation lasts for 1 to 2 shifts. The schedules include 7 AM and 7 PM shifts with weekend rotations every other weekend. Holidays will also rotate among the staff. Patients may include those post-operative for hips, knees, and shoulders. Other diagnoses may include pneumonia and congestive heart failure requiring extra rehabilitation before returning home. Nurses should attempt to initiate IVs before the IV team is involved. Respiratory therapy is available 24/7. Pharmacy services are also available around the clock.
