• provide direct nursing care to long-term care residents.
• Assess residents’ health status and monitor changes in condition.
• Administer medications and treatments as ordered.
• Perform and document head-to-toe assessments, vital signs, and nursing interventions.
• Develop, implement, and update care plans.
• Monitor residents for changes in condition and notify the physician/NP as needed.
• Manage wounds, IVs, feeding tubes, catheters, and other nursing needs as applicable.
• Assist with admissions, transfers, and discharges.
• Coordinate care with CNAs, LPNs/LVNs, physicians, therapists, and other healthcare staff.
• Supervise and delegate tasks to nursing assistants and other staff according to state regulations.
• Maintain accurate documentation in the EMR/charting system.
• Communicate with residents and families regarding care, treatment, and changes in condition.
• Follow infection-control, medication-safety, and facility policies.
• Respond appropriately to emergencies and changes in residents’ conditions.
• Participate in shift report and maintain continuity of care.
• Provide compassionate care while promoting residents’ comfort, dignity, safety, and independence.
• Complete required incident reports and regulatory documentation.
• Follow CMS, state, and facility LTC regulations and quality standards.
