Job Requirements

• Active Registered Nurse (RN) license in good standing
• Minimum of 2 years of bedside clinical nursing experience
• ICU experience or strong clinical skills appropriate for an LTAC setting
• BLS certification required prior to the first shift
• ACLS certification required prior to the first shift
• Ability to work effectively in a team-oriented, patient-centered care environment

Responsibilities

• Complete timely nursing assessments and use findings to develop an individualized plan of care
• Collaborate with patients, families, and the interdisciplinary care team to identify teaching needs and care goals
• Provide accurate documentation of assessments, interventions, and patient responses
• Support rehabilitation goals and deliver direct patient care using the nursing process: assess, plan, implement, and evaluate
• Initiate discharge planning from admission and begin patient education early in the care process
• Communicate changes in patient condition to the appropriate care team members to promote continuity and patient safety
• Guide, support, and supervise ancillary nursing personnel as appropriate
• Maintain a safe, organized care environment while following established hospital policies and procedures
• Contribute to a positive unit culture through teamwork, professionalism, and calm communication
• Participate in ongoing professional development and perform additional duties as assigned
