Job Requirements

• Active Registered Nurse (RN) license in the applicable state
• Minimum of 2 years of bedside clinical experience in acute care, med-surg, or LTAC nursing
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong communication, assessment, and documentation skills
• Ability to work effectively in a fast-paced, patient-centered healthcare environment

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of admission
• Develop and implement individualized plans of care for medically complex patients
• Provide patient and family education based on identified health care needs
• Communicate patient status updates to the interdisciplinary care team to support continuity of care
• Document assessments, interventions, and patient responses accurately and promptly in the medical record
• Participate in rehabilitation programming and interdisciplinary team conferences
• Initiate discharge planning and teaching from the day of admission
• Supervise and support ancillary nursing personnel as needed
• Promote a safe, organized, and positive LTAC care environment
• Contribute to strong team collaboration and quality patient outcomes
