Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 2 years of bedside clinical experience
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong communication, documentation, and teamwork skills
• Ability to work effectively in a patient-centered LTAC environment

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of admission
• Develop and carry out individualized plans of care in collaboration with the interdisciplinary care team
• Document assessments, interventions, and patient responses accurately and on time
• Support patient rehabilitation, recovery, and education throughout the course of care
• Initiate discharge planning and patient teaching from day one
• Communicate changes in patient status to physicians, charge nurses, and team members
• Provide direction and supervision to ancillary nursing personnel as appropriate
• Promote a safe, organized, and positive environment for patients, families, and staff
• Participate in unit teamwork, quality care initiatives, and ongoing professional growth
