Utilizing Jean Watson's Theory of Caring and Relationship Based Care, the RN assesses, plans, implements, and evaluates care for all patients assigned to his/her care. The RN makes determinations regarding the progress of the patient toward goals and utilizes a therapeutic professional relationship with the patient/family and interdisciplinary team to facilitate the promotion, maintenance, and restoration of health. Care is provided according to the specific physiologic, developmental, and psychosocial needs of the patient. The RN shall be considered competent when he/she consistently demonstrates the ability to transfer scientific knowledge from social, biological, and physical sciences in applying the nursing process. Provides care that is consistent with the interdisciplinary plan of care and grounded in the principles of Relationship Based Care. Assesses patient physical and psychosocial condition on an ongoing basis and formulates a nursing diagnosis through observation of the client's physical condition and behavior. Forms a care plan in collaboration with the client, ensuring that direct and indirect nursing care services provide for the client’s safety, comfort, hygiene, protection, and for disease prevention and restorative measures. Performs essential nursing skills, explains health treatments to the client and family, and teaches them how to care for the client's health needs. Evaluates the effectiveness of the care plan through observation of the client’s physical condition and behavior, and modifies the plan as needed. Demonstrates a current working knowledge and safe use of equipment, instrumentation, and medications. Delegates tasks to care team members based on their legal scopes of practice and supervises nursing care provided by the care team. Acts as the client’s advocate, initiating action to improve health care or to change decisions or activities that are against the interests of the client. Provides culturally competent, age-appropriate, ethical care within legal standards of practice. Coordinates patient care activities with nurses, physicians, allied health professionals, and ancillary care team members. Utilizes effective communication skills to facilitate safe patient handoffs. Documents nursing care per hospital policies. Utilizes manual and automated information systems to improve efficiency and productivity. Accountable to meet unit and organizational goals regarding patient satisfaction. Contributes to positive patient care experiences through individual interactions with patients and families. Assists in the resolution of complaints with the goal of immediate resolution if possible. Performs other duties as assigned.
