A professional caregiver who plans, provides, and directs patient care and teaching. Utilizes the nursing process in accordance with patient care standards and physician orders. Acts as a liaison between physicians, patients, and families. Collaborates with other healthcare members. Assists in the orientation process of new personnel. May participate in health center and department committees. Demonstrates the knowledge and skills necessary to provide care appropriate to age groups. Conducts a comprehensive physical and psychosocial assessment of the patient. Assesses safety and educational needs. Participates in establishing a collaborative plan of care based on assessment data. Communicates the plan of care as appropriate. Implements and evaluates the plan of care based on identified patient goals. Modifies the plan of care based on reassessment of the patient’s status. Completes all documentation according to established standards. Utilizes established documentation tools appropriately. Assesses the readiness of the patient and significant others to learn. Implements the individualized education plan according to the need. Actively participates in nursing department or unit-based initiatives. Provides age-appropriate care for the specific population served. Performs all functions according to established policies and procedures. Demonstrates a commitment to providing an excellent service experience.
