The focus is on creating a caring environment that promotes healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and departmental policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is fostered through collaboration among nursing staff and across other disciplines. Responsibilities include ensuring and providing direct patient care activities on the unit. This encompasses the provision of nursing rounds, assessments, and treatments. Facilitation of admission, transfer, and discharge activities is also a key component. Provision of patient and family-centered education is essential. Care coordination and individualized treatment planning are conducted as applicable. The RN performs comprehensive assessments according to the scope of practice and hospital policies. Cultural, age, and developmental factors are considered in patient assessments. The RN identifies patient risk factors and presenting complaints. Patient health information is obtained from family or others as needed. Objective and subjective data are collected during assessments. The RN verifies initial admission assessments made by other healthcare team members. A prioritized problem list is developed based on input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family per the medical plan of care. Patient care therapies and procedures are fully implemented with clinical autonomy. The RN explains procedures, treatments, and processes to patients and families. Initiatives are taken to alleviate patient stress and discomfort. Changes in patient conditions are recognized and communicated appropriately. Ongoing assessments and responses to therapies and procedures are conducted and documented. The RN delegates tasks appropriately and oversees team members performing patient care activities. The accuracy and completeness of delegated care provided by team members are monitored. Educational needs of patients are identified and addressed according to their comprehension levels. Facilitation of the patient discharge process is executed by following timely discharge orders. Input is provided into patient/nurse assignments considering staffing and patient acuity. Active participation in unit-based activities supporting organizational values is expected. A positive group dynamic and healing environment are promoted through actions and words. Nursing excellence is assured in accordance with accepted standards and evidence-based practices. Participation in quality improvement activities and monitoring of documentation is a responsibility. A holistic perspective regarding patient care is maintained throughout.
