Reports and is accountable to the Nurse Manager/Director. Responsible for coordinating and delivering patient care utilizing the nursing process. Participates in identifying and meeting learning needs for self. Attends education programs based on identified learning needs. Uses patient education materials relevant to the patient population with assistance. Assesses readiness to learn of the patient, family, and caregivers. Recognizes developmental levels affecting the ability to learn. Identifies expected patient outcomes for care evaluation. Seeks guidance as needed in revising the plan of care. Participates in discussions with members of the interdisciplinary team regarding patient care evaluation. Communicates relevant information to promote continuity of care.
