- Position: RN Case Manager - Hospice
- Shift: Days: 8:30 AM - 5 PM (Flexible schedule based on patient availability)
- Weekends: One weekend per month
- Minimum Experience: 1 Year of Hospice Nursing Experience
- Certifications: BLS/BCLS Required
Position Summary:
- Admit patients to service and assess their physical, psychosocial, environmental, safety, and developmental needs
- Develop individualized care plans based on thorough assessments
- Document observations and interventions related to each patient’s environment
- Coordinate care with a multidisciplinary team to ensure holistic support
- Participate in the hospice program's quality assessment performance improvement program
- Conduct follow-up visits as needed to ensure ongoing care and support
Territory Details:
- Areas Include: Oakland, Alameda, San Leandro, Ashland, Castro Valley, San Lorenzo, Hayward, Union City, Fremont, Newark, Milpitas, and neighboring cities as necessary
