Hospice Registered Nurse (RN)
Job Summary
Join Grand County Hospice, a leading provider of compassionate, high-quality end-of-life care, located in Moab, Utah. We are dedicated to treating every patient with dignity and respect, primarily providing care in their preferred place of residence, usually their home.
We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to become an integral part of our interdisciplinary team. In this role, you will deliver skilled nursing care and essential support to terminally ill patients and their families, ensuring alignment with individualized care plans and the core philosophy of hospice.
Key Responsibilities
- Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
- Provide effective pain and symptom management consistent with the hospice plan of care.
- Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
- Collaborate effectively with the interdisciplinary team, including physicians, social workers, chaplains, and volunteers.
- Participate in on-call rotation as scheduled.
- Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.
Required Qualifications
- Minimum of 3 years of nursing experience.
- Valid driver’s license and reliable transportation for home visits.
Preferred Qualifications
- Prior experience in hospice, home health, or palliative care.
- Experience with Hospicesoft or Careficient EMR systems.

Shift Details
- Shift Type: AM
- Shift Duration: 8 hours
- Days per Week: 5 days
- Contract Length: 13 weeks
- Start Date: August 20, 2026
- On-Call Requirements: 1-3 shifts per week, including 3 weekends during the 13-week contract. Must be able to respond to patient needs within 45 minutes when on call.
- Holiday Requirements: Ability to work holidays as required.
Licensing Requirements
- Current Registered Nurse (RN) license in the state of Utah or a compact state RN license.
- Current Basic Life Support (BLS) certification.
