This role involves admitting patients onto services and coordinating and integrating services to ensure continuity of care. The RN facilitates the achievement of quality, clinical, and cost outcomes. Negotiation and procurement of services and resources needed by the patient or family are essential functions. Creating opportunities and systems to enhance outcomes is a key responsibility. The nursing process of assessment, planning, and implementation of care is a fundamental aspect of the role. Patient and family instruction and evaluation are conducted through an inter-disciplinary team approach. This position may start as a Visiting RN and transition to Case Manager. Essential duties include providing skilled nursing and coordinating care to patients and family members in a residential setting. The RN observes, assesses, evaluates, and plans interventions in relation to patients' physical and psychosocial statuses. Changes in condition and reactions to treatments or medications are monitored throughout the duration of contact with patients. The RN oversees care provided by Home Health Aides and evaluates compliance with the plan of care. Continuity of care is assured through effective communication and information exchange with other disciplines and agencies. Quality care and appropriate symptom management are provided for positive clinical outcomes. Promoting optimal health and quality of life through teaching, counseling, and appropriate interventions is a focus. Accurate, current, and complete patient records are maintained in accordance with agency standards. Participation in team scheduling coordination and on-call rotation is required to meet patient care needs. The RN participates in quality improvement activities and attends in-service training as assigned. Other duties may be assigned by the supervisor. Recent acute care, home care, or hospice experience is preferred. Clinical skills such as venipuncture, catheterization, wound care, and parenteral therapy are valuable. Experience with Homecare Homebase (HCHB) is strongly preferred. Holiday coverage is required.
