Job Summary:
Hospice Nursing providing compassionate end-of-life care in a residential setting. The RN Case Manager offers skilled nursing, care, and case management for assigned patients in their place of residence, ensuring continuity of care through an interdisciplinary team approach. The position involves patient assessment, planning, and implementation of care, including patient and family instruction, with opportunities to transition from a Visiting RN role.

Job Details:

  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
  • Required Qualifications: Valid RN license in the State of Montana, a valid driver's license, appropriate auto insurance, reliable transportation, and minimum 2 years of recent acute care, home care, or hospice experience including clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy. HCHB experience is strongly preferred. Position start date is 09/21/2026 and end date is 12/21/2026.
  • Preferred Qualifications: Experience in hospice care, BSN degree preferred.
  • Additional Information: Provides skilled nursing and coordinates care to hospice patients and family members in a residential setting through a team approach, based on systematic evaluation authorized by a physician. Oversees care provided by Home Health Aides and CNAs, evaluates compliance with care plans, and maintains accurate patient records. Participates in team scheduling, quality improvement activities, and in-service training.
