Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum one full year of recent adult home health or hospice experience License: Must have an active license in hand Certifications: CPR and BLS Must-Have: - Proficiency in Homecare Homebase EMR and OASIS - Must be willing to travel up to 75 miles from the assigned care center - Reliable transportation, current driver's license, and agency-required liability insurance Description: The role requires providing home visits and attending periodic office meetings. The average daily caseload is approximately 58 patients. Weekly productivity expectations are set at 30 points. Visit types, documentation, travel, and other activities have different point values. Contractors typically see 5-10 patients per day, totaling approximately 68 points. No guaranteed hours are provided. In case of a low patient census, the contractor should contact the Account Manager about floating to a nearby clinic for additional hours. Case types may include start of care, wound care, follow-up visits, and other adult home health or hospice services. Candidates with hospice experience will be preferred, but experience in both home health and hospice is also considered. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing or follow-up patient care may result in a cancellation. Contractors are expected to accept every patient on their schedule, regardless of distance from previous appointments. Travel between patient homes is reimbursable, but travel to the first patient and from the last patient is not billable. First-time travelers are not accepted; recent specialty experience is required.
