Description:
This is a backfill position.
The contractor will be responsible for providing hospice care to patients in their homes.
The typical schedule is Monday through Friday from 8:00 AM to 5:00 PM.
One weekend per month may be required.
There will be minimal on-call responsibilities.
Travel will be required within a specified territory.
Contractors can expect to travel approximately 200 to 400 miles per week.
Weekly productivity expectations are between 25 to 30 units.
Average daily caseload will be 5 to 8 patients.
Visit types may include start of care, wound care, follow-up visits, and other hospice services.
Documentation must be completed within 24 hours of each appointment.
Delays in documentation that impact patient care may result in cancellation of assignments.
There are no guaranteed hours with this contract.
Clinicians must accept all patients on their schedule regardless of distance between appointments.
Reliable transportation is required for daily travel to patient homes.
Candidates must have experience in both adult home health and hospice care to be considered.
