Must-Have: Proficient in HomeCare HomeBase (HCHB) and OASIS, used within the last 6 months.
Description:
Provides, coordinates, and directs the provision of home nursing care according to physicians' orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits are required.
Periodic office meetings are part of the role.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
The average number of patients per day is between 5 to 10, equating to approximately 6-8 points based on weekly productivity expectations.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be willing to travel up to 75 miles from the assigned care center for patient home visits.
Mileage will be reimbursed at the IRS standard rate.
The contractor will accept every patient on their schedule regardless of the distance from the previous appointment.
Documentation must be completed within 24 hours of the appointment.
Expectations include hitting weekly productivity points totaling 30, equating to 40 hours of work.
There are no guaranteed hours with this contract.
All overtime must be pre-approved.
Failure to meet standards may result in cancellation of the contract.
