You will manage an assigned caseload, developing personalized goals of care and ensuring comprehensive support for patients navigating end-of-life journeys. Key Responsibilities
- Provide compassionate, holistic nursing care to hospice patients.
- Manage an assigned caseload of patients across diverse settings (private homes, nursing homes, group homes, assisted living facilities).
- Lead and collaborate effectively with the interdisciplinary care team.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Specialize in symptom management and pain control for hospice patients.
- Educate patients and caregivers on disease progression, care techniques, and personal goals of care.
- Provide emotional support and guidance to patients and families.
- Document patient care accurately and timely.
Required Qualifications
- Current Registered Nurse (RN) license.
- Minimum of Not specified years of clinical nursing experience.

Shift Details
- Shift: Day Shift (AM)
- Hours: 8 hours per shift (8:00 AM - 5:00 PM)
- Days: Monday - Friday
- Call Requirements: Occasional back-up for on-call.
- Weekend Rotation: Occasional back-up for weekends.
- Contract Length: 12 weeks
- Assignment Start Date: 2026-09-14
Licensing Requirements
- Active and unrestricted Registered Nurse (RN) license in the State of Colorado.
