Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours Experience: Minimum of 1 year as an RN in acute care, oncology, long-term care, hospice, or home health. License: Current nursing license in the state of employment. Certifications: BLS (American Heart Association). Must-Have: - Knowledge of terminally ill patients and their families. - Ability to apply knowledge of the special needs of hospice patients and families. - Excellent patient assessment skills. - Knowledge of regulatory guidelines such as Medicare and Medicaid. - Strong oral and written communication skills. Description: The Registered Nurse (RN) will provide direct care services to hospice patients. Supportive care will be provided to the patient's family. The RN will manage an interdisciplinary group of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff. The goal is to enhance the quality of hospice care. The RN assures the highest quality of care is provided and documented in accordance with the Plan of Care. Initial and ongoing assessments of the patient will be performed. The RN will help develop and implement the Hospice Plan of Care as prescribed by the physician. Collaboration with other members of the Interdisciplinary Group is essential to provide all-inclusive services. Physical, psychosocial, educational, emotional, and spiritual services will be delivered to achieve high-quality care. Changes in the patient's condition will be reported to both the Attending Physician and Hospice Physician. Accurate and updated medical records will be maintained, reflecting all interdisciplinary notes and assessments. Infection control measures and standard precautions will be practiced. The RN will monitor the scheduling of all staff. Participation in the Quality Assessment and Performance Improvement Program may be required. The RN promotes the organization’s core values consistently. This position is classified as safety-sensitive. The RN will be responsible for the education of the patient, family, or caregivers regarding needs identified on the Plan of Care. Supervision of licensed practical nurses and hospice aides providing delegated nursing services is required. Maintaining close contact with the patient’s family and providing information and support is essential. The RN acts as the primary liaison between the physician, patient/family, and hospice team. A high level of knowledge regarding pain management and symptom management in the dying process is expected. Routine supervisory visits with all other team members are required at least monthly. Ongoing review and follow-up on assigned patient clinical records will ensure that quality services are provided. Compliance with policies, procedures, and regulations will be maintained. The RN will participate in staff meetings and perform other assigned activities.
