Key Responsibilities- Serve as a hospice case manager providing care to patients in the home health setting
- Manage and coordinate patient care plans across the continuum of hospice services
- Conduct patient assessments and implement individualized care interventions
- Collaborate with interdisciplinary team members to ensure quality end-of-life care
- Document patient progress and clinical findings accurately and in a timely manner
Required Qualifications- Active Registered Nurse (RN) license; Virginia RN license required to be in hand at time of submittal, or proof of compact license must be provided
- Hospice experience in the home health setting is required
- Specialty experience within the last year is required
- BLS certification through the American Heart Association (AHA) required
- One supervisory reference required at time of submittal
- Local candidates are welcome and accepted
Preferred Experience / Skills- Experience with electronic health record documentation
- Strong communication and care coordination skills within an interdisciplinary team environment
- Ability to manage a patient caseload independently in a field-based setting
