Job Summary:
Hospice Nursing in a home-based care environment. This role involves providing compassionate end-of-life care to patients in various settings such as private homes, nursing homes, group homes, or assisted living facilities. The primary nurse will manage a caseload of patients, collaborate with an interdisciplinary team, and serve as an educator for patients and caregivers. The position emphasizes assessment, symptom management, and supporting patients' personal goals of care.

Job Details:

  • Schedule: Weekday schedule of Monday through Friday, from 8:00 a.m. to 5:00 p.m., with occasional backup for weekends and on-call responsibilities.
  • Required Qualifications: Valid nursing license, diploma or degree in nursing, one year of relevant experience, and ability to work in the specified location.
  • Preferred Qualifications: Experience in hospice or palliative care, strong assessment and critical thinking skills, and ability to provide patient education.
  • Float: Float rotation may be required.
  • Additional Information: This position involves delivering hospice care in patients' homes and guiding families and caregivers through the care process. The role requires a compassionate approach and strong clinical judgment to support patients and families during challenging times.
