The RN will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient's plan of care and hospice philosophy. Key responsibilities include performing comprehensive assessments of patients' needs and coordinating individualized care plans. Providing pain and symptom management consistent with the hospice plan of care is essential. Education and support for patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy will be provided. Collaboration with an interdisciplinary team including physicians, social workers, chaplains, and volunteers is required. Participation in on-call rotation is necessary, with the expectation to cover 1-3 shifts per week, including weekends during the contract period. Timely and accurate documentation in compliance with state and federal policies is expected. Experience with hospice software is preferred.
