Description

Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Care Duration: 14 Weeks Shift: 40 Hours Per Week Hours per Shift: Varies Experience: Minimum years of hospice experience required, including on-call and admissions experience. License: Active RN license required. Certifications: Certification in Hospice and Palliative Care preferred. Must-Have: Experience using electronic medical records. Description: Identify the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. Individualize the patient's plan of care based on the needs identified. Document problems, nursing assessments, appropriate goals, care provided, care interventions, and patient and family outcomes achieved from intervention and care provided. Assess all aspects of the patient's pain and develop an individualized pain management plan. Anticipate, prevent, and treat undesirable symptoms or secondary symptoms. Participate in the interdisciplinary team (IDT) meeting to establish, review, and revise the plan of care for each patient. Exchange information, review problems, and share professional support during IDT meetings. Maintain up-to-date patient records to ensure that problems, plans, interventions, and goals are accurately stated and changes are reflected as they occur. Teach appropriate interventions, medications, effects, and side effects to patient, caregivers, and volunteers as appropriate. Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patient and family. Be responsible for maintaining electronic medical records, assuring accuracy, completeness, and compliance with regulations, certification standards, legal and ethical standards. Identify and address comfort care needs. Support, instruct, and educate the patient, family, and caregiver. Coordinate all patient and family services. Maintain the dignity of the dying patient. Support the patients and family's unique spiritual and cultural beliefs. Provide holistic, family-centered care across treatment settings to improve the quality of life. Consult and collaborate with the interdisciplinary team and others involved in patient care. Provide care consistent with national standards of practice for hospice programs.
