PDTXperts is seeking a travel nurse RN Home Health Med Surg for a travel nursing job in Everett, Washington.

Qualifications
Active RN license in the state where services will be provided.
Graduate of an accredited nursing program.
Minimum 1 year of Home Health experience within the past 3 years (required).
Current CPR/BLS certification.
HCHB (Homecare Homebase) EMR experience required.
Reliable personal transportation required.
Strong clinical assessment, communication, and documentation skills.
Key Responsibilities
Conduct patient assessments and develop or implement individualized nursing care plans.
Provide skilled nursing care, wound care, medication management, and patient monitoring as appropriate.
Complete OASIS assessments and other required clinical documentation when applicable.
Educate patients and caregivers on disease management, medications, and treatment plans.
Coordinate care with physicians and the interdisciplinary team.
Maintain accurate and timely documentation using HCHB EMR.
Follow applicable state regulations and Home Health standards.
