Bachelor's degree in nursing (BSN) required.
EXPERIENCE / TRAINING:
One year of clinical nursing experience required (preferably with geriatric populations) with at least 1,000 hours in an
acute care hospital.
LICENSES / CERTIFICATIONS:
Required: Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN )
Current Basic Life Support Provider (BLS) issued by AHA.
JOB SUMMARY: Under the supervision of the Community Nursing Supervisor/SCN and with direction from the program's
Clinical Supervisor, provides case finding, consultation, health-focused assessment/care coordination
services and care planning services to adults with disabilities, older adults and their families in various
programs. Provides services in home and community-based settings in order to preserve health,
maintenance of function and continued independence through assessing needs, monitoring quality of care,
and providing education, advocacy, and support. Provides nursing consultation and guidance to
departmental staff. This position requires flexibility of hours.
ESSENTIAL JOB FUNCTIONS:
1. Utilizes professional skills and competency with minimal supervision needed, providing health-focused care
coordination, comprehensive nursing assessments and care plan development and certification of level of care for
older adults and adults with disabilities in accordance with program mandates and established clinical practices.
2. Provides counseling and supportive interventions consistent with scope of practice.
3. Independently schedules time in order to meet all client, program, departmental and hospital requirements in a timely
and organized fashion by organizing workload, setting appropriate priorities, and completing thorough and timely
documentation.
4. Helps to ensure high quality care is being provided to program clients by unlicensed caregivers at state-approved &
licensed Assisted Living Waiver facilities through coaching and education as part of the care plan review with staff.
5. Demonstrates excellent fiscal management by maximizing use of available informal and community resources before
relying on purchased services by care coordination programs, if applicable. Ensures adherence to spending
guidelines by managing purchase of service funds and promotes equitable fund allocation across the caseload.
Additionally, meets all productivity standards and serves as a good steward of hospital, departmental and program
resources.
6. Participates and provides leadership in at least one departmental committee.
7. Actively participates in departmental quality assurance activities including utilization review, maintains and
participates in developing quality standards and provides guidance and consultation to other staff.
8. Represents the department in the community through activities such as providing intake and referral services at
community partner sites, participating in developing professional alliances and health fairs and other outreach events,
as assigned.
