Key Responsibilities- Prescribe, coordinate, and deliver patient care through the nursing process while maintaining the standards of professional nursing
- Conduct comprehensive initial and ongoing assessments covering physical, psychosocial, environmental, and community resource factors across the lifespan
- Initiate, evaluate, and modify Plans of Care as needed; establish discharge plans in collaboration with patients, physicians, other disciplines, and caregivers
- Complete OASIS data and all documentation in accordance with current Federal, State, and Agency regulations
- Assess educational needs of patients, families, and caregivers and implement appropriate teaching strategies
- Perform home visits as well as some telehealth/virtual visits; supervise CNAs and coordinate care with other healthcare team members as assigned
Required Qualifications- Active Illinois RN license (must be active at time of submission)
- Home Health experience required
- Active BLS certification or willingness to obtain prior to start
- Valid driver's license and current auto insurance required
- COVID-19 and Influenza vaccinations required; declinations will not be approved
- Local candidates preferred; candidates located outside the service territory may not be considered
Preferred Experience / Skills- OASIS and start-of-care experience strongly preferred
- Strong Med/Surg background combined with some Home Health experience may be considered
- Familiarity with telehealth or virtual visit platforms
- Experience with a broad range of home health services including wound/ostomy care, infusion management, and chronic disease management
