Schedule / Job TypeContract | Days | Monday-Friday, 8:00 AM-4:30 PM (5×8-hour shifts) | Onsite
Key Responsibilities- Serve as office-based Patient Care Manager for a home health setting, overseeing daily operations and dashboard management
- Review orders, coordinate with physicians and clinicians, and manage new patient admissions
- Assist with scheduling coordination as needed within the office environment
- May be required to assist with patient visits outside of the office on an as-needed basis
- Participate in on-call rotation, approximately once per week as needed
- Work within assigned system platforms; Homecare Homebase (HCHB) experience required through 10/1/2026; Epic experience required after 10/1/2026
Required Qualifications- Active South Carolina RN License (or compact equivalent as applicable)
- BLS Certification required
- Minimum of 2 years of experience: at least 1 year of general RN experience plus at least 1 year of home health experience (first-time travelers accepted)
- Patient population: Adults through Geriatrics
- Candidates without Homecare Homebase (HCHB) experience must complete an online HCHB course prior to start
- Local candidates are welcome and accepted
Preferred Experience / Skills- Prior experience with Homecare Homebase (HCHB) electronic health record system
- Familiarity with Epic EMR platform
- Background in home health office management or care coordination roles
