Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Cordova, TN 38018
Schedule: Monday–Friday, 8:00 AM–4:30 PM

Weekly Gross Pay : Up to $2,562.00
Hours: 40 hours/week
Coverage Area: Shelby, Fayette & Tipton Counties
Mileage: Reimbursed between patient visits
On-Call/Weekend: None
Productivity: 6 visits/day / 30 points per week
Scrubs: Royal Blue

Position Summary:

  • We are seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, high-quality hospice care to patients in their homes.
  • The RN will be responsible for case management, admissions, care coordination, and preparation for interdisciplinary team (IDT) meetings.

Key Responsibilities:

  • Manage a caseload of hospice patients and coordinate individualized plans of care.
  • Conduct hospice admissions, assessments, and ongoing skilled nursing visits.
  • Coordinate care with physicians, patients, families, and the interdisciplinary team.
  • Prepare accurate documentation and patient updates for IDT meetings.
  • Monitor patient condition and communicate changes in status to the appropriate care team.
  • Maintain timely and accurate clinical documentation in HCHB.
  • Provide education and support to patients and families regarding hospice services and the plan of care.

Required Qualifications:

  • Current, active RN License.
  • Previous Hospice nursing experience – required.
  • HCHB experience – required.
  • Strong clinical assessment, documentation, and care coordination skills.
  • Ability to travel throughout the assigned coverage area.
