Position: RN Home Health
Contract: 13 weeks with possible extension
Shift: 8:00 AM - 5:30 PM, 5x8s
Location: 1900 Silver Cross Blvd, New Lenox, IL 60451
Pay: Up to $56/hr 
Note: Local candidates are strongly preferred 

Job Description

• Prescribes, coordinates,s and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other health care workers as assigned.
• Provides comprehensive initial assessments to all clients and, on subsequent visits,s continually assesses Physical, psychosocial, environmental, al and community resources available, utilizing the principles of growth and development over lifespans.pan
• Initiates Plan of Care, re-evaluates the need for care, a nd modifies Plan of Carordered/neededeeded.  Establishes discharge plans with patient, physician, other disciplines and caregivers.
• Completes OASIS data
• Provides patient care using current Federal, State, and Agency regulations
• Assesses educational needs of patient, families, caregivers and implements appropriate teaching strategies
• Completes documentation according to regulatory guidelines.
• Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
• Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct
