Home Health RN – Case Manager
Location: St. Albans, Vermont Shift: Monday–Friday, 8:00 AM–4:00 PM Specialty: Home Health RN / Case Manager
Position Details
- Provide home health nursing care and manage assigned patient caseloads.
- Complete OASIS assessments and documentation accurately and on time.
- Develop, implement, and evaluate individualized plans of care.
- Conduct patient visits, coordinate care, and supervise LPNs and home health aides.
- Provide patient and caregiver education.
- Perform IV care, wound VAC management, and complex wound care.
- Coordinate with physicians, therapists, social workers, and other care team members.
- Maintain accurate documentation using McKesson processes.
- Coordinate patient discharges, transfers, case conferences, and quality audits.
Requirements
- RN license eligible to practice in Vermont.
- Home health nursing and case management experience.
- OASIS documentation experience.
- Experience with IV therapy, wound VACs, and complex wound care.
- Strong care coordination, patient education, and documentation skills.
- Ability to manage a patient caseload and conduct home visits.
