Job Summary
Home Healthcare Nurse in California responsible for planning, organizing, and directing home care services to meet patients' needs within their homes. Manages a patient caseload effectively and provides patient care independently, collaborating with other core disciplines to meet care goals. Acts as a resource for patients, families, and care team members.
Job Details
- Schedule: Rotating shift; 8 hours per shift; minimum 40 hours per week.
- Required Qualifications: Bachelors Degree in Nursing (BSN); two years of experience in home care or acute care setting; Registered Nurse (RN) licensure in the state of practice; Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor; Valid Drivers License and at least 21 years of age or older; current automobile insurance and reliable vehicle.
- Essential Functions:
- Conducts initial and ongoing comprehensive assessments of patients needs at designated times.
- Performs nursing assessments of assigned patients.
- Educates patients and families on healthcare and nursing needs, including written materials as appropriate.
- Identifies issues requiring follow-up and reports problems or abnormal findings.
- Implements and adjusts plans of care aligned with patient goals.
- Coordinates care with other disciplines and incorporates team input.
- Documents all patient care activities, responses to treatment, and therapeutic interventions promptly and legibly according to policies.
- Updates nursing plans and medication profiles as changes occur.
- Ensures compliance with legal, privacy, and documentation standards, including HIPAA and code of conduct.
- Provides on-call services as required based on location and agency needs.
- Performs other related duties as assigned.
