Registered Nurse (RN)

Job Description

This position requires a Registered Nurse with a strong background in home care and case management. The ideal candidate will provide clinical services to patients across various age groups, from infants to seniors, in their homes.

Responsibilities

Assessing patient conditions and implementing appropriate care plans.
Providing ongoing education and support to patients and their families.
Conducting the admissions process and managing cases in alignment with established policies and standards of practice.
Utilizing thorough and timely electronic documentation and interdisciplinary communications.

Qualifications

A minimum of 1 year of certified home care experience.
At least 1 year of case management experience.
Current licensure as a Registered Nurse in the state of Massachusetts.
Reliable, independent means of transportation for making home visits.
Willingness to work weekends, holidays, and on-call shifts as per departmental needs, with weekend rotations occurring once every six weeks.
