Start ASAP for 13 weeks M-F,
8am-5pm
Weekend rotation: yes preferred if possible
Coverage area: Jackson County May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address RN productivity expectation for 40 hrs = 28 - 32 points/wk SOC 2.5, ROC 2.0, Discharge (OASIS) 1.5, Recertification- 1.5, Routine visits- 1
Must have recent Home health exp 2025-26
HomeCare HomeBase experience required Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.
