Position Summary: 

We are seeking an experienced Registered Nurse (RN) – Home Health to provide high-quality, patient-centered nursing care in patients’ homes. The RN will assess patients, develop and implement individualized care plans, administer medications and treatments, and coordinate care with physicians, caregivers, and other members of the healthcare team.

Responsibilities:
 

• Provide skilled nursing care to patients in their homes
• Perform assessments and develop individualized care plans
• Administer medications and treatments
• Monitor patient conditions and report changes
• Educate patients and families on disease management and care
• Collaborate with physicians and interdisciplinary healthcare teams
• Maintain accurate clinical documentation

Requirements:

  • Active OR State, unrestricted certification/licensure as RN
  • Oasis experience is required.
  • 1 year of Skilled Home Health Experience
  • Active BLS from AHA.
  • COVID card or Medical / Religious Exemption.
