Description: This position involves providing home health services to patients in their homes daily. The nurse will be responsible for driving to patients' homes and attending periodic office meetings. Average daily caseload is 5 to 8 patients. Weekly productivity expectation is 30 points. Visit types may include start of care, wound care, follow-up visits, and other adult home health services. Documentation must be completed within 24 hours of the appointment with the patient. All clinicians must be proficient in the specified EMR systems. Reliable transportation, a current driver's license, and agency-required liability insurance are required. Mileage is reimbursed at the standard rate, excluding travel between the first and last patient. The nurse must be open to potential on-call responsibilities. Travel may be required up to 75 miles from the home clinic. Previous specialty experience is required, and first-time travelers are not accepted. Declining patients may lead to cancellations, so acceptance of each patient on the schedule is expected. Delayed and incomplete documentation impacting billing will also result in cancellations.
