License: Active nursing license required. Certifications: CPR and BLS required. Must-Have: Proficiency in Homecare Homebase (HCHB) and OASIS within the last 6 months. Description: The Registered Nurse will provide home visits to patients in their residences. This position requires driving to multiple locations daily. The clinician will attend periodic office meetings. The average daily caseload is 5 to 8 patients. Weekly productivity expectation is 30 points, which equates to approximately 40 hours of work. Visit types may include start of care, wound care, follow-up visits, and other adult home health or hospice services. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will be addressed. The clinician is expected to accept all scheduled patients regardless of the distance from previous appointments. Travel between patient homes is reimbursed, while travel to the first patient and from the last patient is not billable. Weekly travel mileage of up to 100 miles is anticipated. On-call responsibilities may be required, with potential rotations among staff. Reliable transportation and appropriate insurance are necessary.
