Job Title: Home Health RN Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: M-F Hours per Shift: 8am-4pm Experience: Minimum 2 years License: Current Nursing License Certifications: OASIS experience required, Hospice coverage while on call required Must-Have: - OASIS experience - Hospice experience Description: Utilizes the nursing process to assess patient/caregiver needs in order to provide quality care to assigned patients. Develops and communicates a realistic plan of care for each patient in accordance with evidence-based practice and regulations. Serves as Case Manager for assigned patients and actively manages caseloads and visit times. Effectively fills out and completes documentation in accordance with agency policies and regulations. Demonstrates knowledge of and stays up-to-date on changes to documentation requirements. Implements medical plan of care, nursing interventions, and procedures to ensure safe and effective quality of care. Stays updated on a variety of High-Tech skills, such as IV use and maintenance and complex wound care. Works collaboratively with LPNs to ensure that the plan of care is followed and patient needs are met. Evaluates various aspects of the implemented plan of care to ensure overall quality of care delivered. Maintains up-to-date and accurate documentation of nursing care provided for effective integration of information. Follows agency protocol with processes including visit completion and follow-up notes. Performs patient and caregiver education to ensure informed patient decisions can be made. Identifies inter-disciplinary needs of the patient and documents patient needs accordingly. Accurately fills out a variety of agency forms in a timely manner. Carries out visits in a time-effective manner. Assesses patients' needs for services, developing and coordinating plans of care with patients/caregivers. Transmits patient incident reports to relevant specialists in a timely manner. Coordinates safe patient discharges and ensures all appropriate paperwork is processed. Coordinates a wide variety of patient care with providers and team members in support of the recommended plan of care. Participates in peer review audits, providing constructive feedback and accepting feedback professionally. Schedules and facilitates case conferences with team members to maximize effective communication. Advocates for patient care issues to ensure sensitivity to each patient's needs.
