Key Responsibilities
Provide skilled home health nursing visits to patients primarily transitioning from hospital or nursing facility settings
Perform and manage complex procedures including wound care, wound vacs, Pleurx drains, and ostomies
Care for patients with diagnoses including Heart Failure, Diabetes, oncology, post-surgical, and orthopedic conditions
Complete OASIS assessments and document all care accurately using EPIC EMR
Maintain a productivity standard of 6 visit-equivalents per day (e.g., admissions count as 2 visits)
Rotate every 5th weekend and every other holiday as part of the standard schedule
Required Qualifications
Active New Jersey RN License required; must be in hand at time of submission - pending licenses are not accepted
Minimum 2 years of recent Home Health experience, with experience within the last 12 months
Strong Med-Surg clinical skills
Proficiency with OASIS and EPIC EMR
Valid driver's license and reliable personal transportation required
Active BLS certification required; must not expire within 30 days of assignment start date
Candidates living within 50 miles of the primary facility are subject to the local rate
Preferred Experience / Skills
Experience caring for oncology, orthopedic, or post-surgical home health patients
Familiarity with Heart Failure and Diabetic patient management in a home setting
Experience with Pleurx drains and advanced wound care modalities
