Description:
This position is for a Home Health Registered Nurse.
The RN will provide care in patients' homes in the designated area.
The role requires visiting patients and performing necessary evaluations and treatments.
The RN should have experience with admission assessments and discharge planning, as well as wound care management.
The RN is expected to manage medication administration and assess patient pain levels.
The position involves visiting up to six patients per day, adhering to productivity standards.
The RN must have their own transportation, as home visits can involve travel.
The patients will include both adults and geriatrics.
The RN will use electronic health records for charting and documentation during visits.
Flexibility is key, as the role may involve travel up to 60 miles per day, which could result in varying drive times.
Vaccination for COVID-19 is required, along with a booster.
The facility cannot accommodate shifts other than 8-hour days.
There are no weekend shift requirements, and no on-call shifts are required.
The RN will work independently with support from interdisciplinary teams as needed.
