Shift & Schedule: 8–10 hour days, 8:00 AM–4:30 PM
Weekly Hours: 36 hours/week
Contract Length: 13 weeks
Call/Weekend/Holiday Requirements: Every 3rd weekend and holiday rotation; on-call every 4th weekend and 1 weekday

Requirements:

• AHA BLS
• Active MI and WI license
• Valid driver’s license
• 2+ years home health experience preferred
• OASIS required
• Associate degree or certificate required
• Bachelor’s degree preferred

Responsibilities

• Average 5 visits/day; up to 6
• OASIS admissions and all time points
• Episode management
• Doctor calls and coordination of care
• Timely EMR documentation
• Wound, IV, port, and lab-draw competencies
• Chronic disease management
