Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Denver, CO 80222

Patient Care Setting: Home Health
Coverage Area: Denver Metro Area
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectation: 30 Points Per Week
On-Call Requirement: Yes, Rotating On-Call Schedule
Weekend Requirement: Yes, Rotating Weekend Coverage
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Business Casual or Scrubs (Color Not Specified)

Position Overview:

  • We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout the Denver Metro Area.
  • The RN will be responsible for comprehensive assessments, case management, OASIS documentation, wound care, medication management, and patient education while working collaboratively with an interdisciplinary healthcare team.

Key Responsibilities:

  • Perform comprehensive patient assessments, Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges, and OASIS documentation.
  • Develop, implement, and manage individualized plans of care in collaboration with physicians, therapists, caregivers, and interdisciplinary team members.
  • Provide skilled wound care services, including wound assessments, dressing changes, and monitoring wound healing progress.
  • Manage medications, perform medication reconciliation, provide medication education, and monitor patient adherence and response to treatment.
  • Perform laboratory specimen collection and coordinate laboratory services as ordered by physicians.
  • Provide catheter management, including insertion, maintenance, assessment, troubleshooting, and patient/caregiver education.
  • Monitor patient status, identify changes in condition, and communicate clinical findings to physicians and appropriate care team members.
  • Deliver disease management education, safety instruction, and self-care training to patients and caregivers.
  • Coordinate care and resources to ensure continuity of services and optimal patient outcomes.
  • Maintain accurate and timely clinical documentation while meeting agency productivity expectations.
  • Participate in rotating on-call and weekend coverage schedules as assigned.

Required Qualifications

  • Active Registered Nurse (RN) License.
  • Previous Home Health RN experience required.
  • Strong OASIS assessment and documentation experience required.
  • Experience with wound care, case management, medication management, and home-based patient care.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently and effectively manage a field-based caseload.
