Job Details

  • Location: West Portland, OR and surrounding areas
  • Position Description: Backfill for Home Health RN, requiring Oregon State licensure, with an expected start date of 10/05/2026, and ending on 01/04/2027
  • Schedule: 8-hour days, Monday through Friday, minimum of 40 hours per week
  • Required Qualifications: Valid Oregon State licensure, BCLS certification, at least 1 year of skilled home health RN experience, valid driver's license, own transportation, and ability to travel within a 50-75 mile radius
  • Certifications: BCLS required
  • Additional Qualifications: Experience with Charting systems such as EPIC, Rover, and Oasis. Skills in adult home health, wound care, medication management, and specific care procedures are required.
  • Responsibilities: Conducting admission assessments, medication administration, wound care, managing infusion therapies, and utilizing pain scales. Providing care for adult and geriatric patients in their homes, following clinical protocols, and documenting care appropriately.
  • Preferred Qualifications: Experience with Oasis documentation, G-tube care, infection control, and disease management is preferred.
  • Additional Information: Candidate must have possession of their own transportation, be willing to work an 8-hour day shift, and complete orientation modules. Travel between nearby facilities may be required; refusal to float may result in contract termination. Candidates must provide two recent references covering one year of employment during the last three years.
